Insurance
Services may be covered in part by your health insurance or employee benefit plan. I will provide a coded statement that can be submitted with a claim form to your insurance company for reimbursement. Please check with your insurance provider as each plan has different guidelines for psychological services and reimbursement.
Payment
Payment is due at the time services are provided by cash, check or credit card.
Cancelation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the full cost of the session.
